Smarty: 3.1.3
Sometimes I faced this problem, but when I refresh the web page the problem is gone. Any helps?

You should update to the latest version. We did have in the past in some conditions side effects which be the cause for your problem as well.

I have updated the latest version 3.1.6, but the problem still exists. Has the bug fixed?

Okay must be a different problem then...

Did you see this randomly or after the template was modified or if caching is enabled after the cache file did expire?

Caching is disabled. It happens after the template was modified.

This seems to be a different error. The html_image plugin is now checking if the image folder is trusted when security is enable,

You must include the image folder to the $secure_dir property of the scurty class.
